All of our medical records are computer based only. Your medical record is a confidential document. It is the policy of Eagle Vale Medical Centre to maintain security of personal health information at all times and to ensure that this information is only available to authorised member of staff. Patients are able to access their health information upon written request. Your regular GP will be notified of this request and check for exemptions. Photo ID must be produced.
